FORMS

The following is a list of required forms for all participants. Your Applicant Portal (linked in your Welcome Email) is customized for you and may contain additional materials.

1. Medical History Form.

2. Applicant Profile, including Veterans Agreement to Cancellation Policy & a copy of your DD214 (or other form of identification verifying your military service).

3. Outward Bound California Liability Release.

4. COVID-19 Proof of "Up to Date" Vaccination. More on our COVID-19 Practices here.

Your application is dependent on timely return of your application materials. Once all items have been received, your course advisor will reach out with further instructions. 

Please note: We are making a significant commitment to our Military Service Members so it is imperative that once you commit, you follow through and attend the course. If you cancel for reasons other than deployment, you will deplete the funds generously provided by our donors and you will have taken a slot from another deserving service member. Our investment in planning, staffing, and provisioning cannot be recovered in the event of short notice cancellations. If you have to cancel due to an unforeseen emergency, please let your Course Advisor know immediately. Given enough time we can most likely find another participant to fill your slot. Please see our Cancellation Policy (included in the Applicant Profile) for further information.
